29 CFR 1910.178(l)(2)(ii): Powered industrial truck training did not consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  On October 31, 2016 and at times prior thereto the employer had not implemented a formal powered industrial truck (forklift) training program at this location that covered truck related topics and workplace related topics, to include unique or potentially hazardous environmental conditions in the workplace that could affect safe operation of the forklift.     Note) Certification of corrective action IS required for this item.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5):  a) On October 31, 2016 and at times prior thereto the employer did not ensure that the powered industrial truck (forklift) operators previous operator training was appropriate for the working conditions at this location.  This would include the use of a boom attachment, material clamp, size and weight of materials handled.    Note) Certification of corrective action is required for this item.
