29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:    On or about October 31, 2016, employees were engaged in bricklaying activities from a scaffold that was not fully decked at the working level.  Employees were thereby exposed to fall hazards of approximately 18 feet.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    a. On or about October 31, 2016, employees of Burroak Masonry Inc. were required to work on an elevated platform in the performance of their duties. These employees were observed working from a scaffold platform greater than 2 feet to the next lower level without the proper point of access as outlined in this OSHA standards. The employees were thereby exposed to fall from heights.    b. On or about October 31, 2016, employees of Burroak Masonry were observed utilizing cross bracing as a means of access to a scaffold platform.      Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

On or about October 31, 2016, each employees engaged in bricklaying activities from a scaffold was not protected from falling by a guardrail system each the working level.  Employees were thereby exposed to fall hazards of approximately 18 feet.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a. On or about October 31, 2016, employees of Burroak Masonry Inc. were required to walk onto an open sided overhang/floor to access a scaffold. These employees were exposed to a fall hazard if approximately 12 feet to the ground.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

On or about October 31, 2016, employees where engaged in overhand bricklaying activities from a scaffold. The area below this work zone was not barricaded or otherwise demarcated to ensure controlled access into the danger zone. Employees were thereby exposed to struck-by hazards from falling tools, material, and / or debris.    b. On or about October 31, 2016, employees where engaged in overhand bricklaying activities from a scaffold. There were no toe boards installed on the scaffolding planks. Employees were thereby exposed to struck-by hazards from falling tools, material, and / or debris.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
