29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: On or about October 31, 2016, employees were engaged in bricklaying activities from a scaffold that was not fully decked at the working level. Employees were thereby exposed to fall hazards of approximately 18 feet. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a. On or about October 31, 2016, employees of Burroak Masonry Inc. were required to work on an elevated platform in the performance of their duties. These employees were observed working from a scaffold platform greater than 2 feet to the next lower level without the proper point of access as outlined in this OSHA standards. The employees were thereby exposed to fall from heights. b. On or about October 31, 2016, employees of Burroak Masonry were observed utilizing cross bracing as a means of access to a scaffold platform. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Employees were thereby exposed to struck-by hazards from falling tools, material, and / or debris. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
