29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training, and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:       694 Central St, Chicopee, MA    On November 16, 2016 and at times prior, the employer had not established a program consisting of energy control procedures, employee training, and periodic inspections to protect employees who performed servicing or maintenance activities.  On October 21, 2016, an employee received a fingertip amputation injury while setting up a   Kirk Rudy Machine while the machine's belts and pulleys were energized and moving.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    694 Center St, Chicopee, MA    On November 16, 2016 and at times prior, the employer had a written Hazard Communication Program and Safety Data Sheets available, but had not trained new employees or temporary employees on the hazardous chemicals in their work areas including, but not limited to, Super-Kleen roller cleaner, or on the location of the safety data sheets.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    694 Center St, Chicopee, MA    On November 1, 2016 at the start of the inspection, the employer had not reported to OSHA that an employee had received a work-related finger amputation injury on October 21, 2016.
