29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Location:  158 Maple Street, Naugatuck, CT 06770  The 14 foot long fiberglass ladder used to access the 3 in 12 pitched roof from the ground did not extend at least three feet above the eave of the roof and the 14 foot long fiberglass ladder used to access the 6 in 12 pitched roof from the 3 in 12 pitched roof did not extend at least three feet above the eave of the roof.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  Location:  158 Maple Street, Naugatuck, CT 06770    The two sections of the 28 foot long fiberglass extension ladder were used as single, individual ladders in order to access the two roof sections of the building and therefore the extension ladder was used for purposed other than which it was designed.
