While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings: a) On or about October 26, 2016, and at times prior, the employer did not ensure that service pits not in use were covered in a manner that would prevent employees from falling 5 feet to the floor below.
