29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  On or about November 2, 2016, at the above addressed site, an employee was exposed to a fall hazard of approximately 19 feet when stringing lights from the basket of a truck mounted aerial lift while not wearing a safety belt and/or a safety harness with a lanyard attached to the boom or basket of that lift.
