29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:  a)    Room 139B Media Preparation;  An Inovel 3212 N95 respirator was hanging from its straps on the wall.   b)    Office 107;  An Inovel 3212 N95 respirator was stored in a lab coat pocket.

29 CFR 1904.40(a): Basic requirement. When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours:  a)    Copies of OSHA 300 logs were NOT provided to the OSHA inspectors during the two days of the onsite inspection.  Copies of OSHA 300A forms WERE provided to the inspectors for 2015, 2014, and 2013.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a)    Microbiology Lab Room 118;  A bottle of PH solution was not labeled.  b)    Microbiology Lab II Room 133B;  A bottle of ETOH was not labeled.  c)    Room 114A;  A bottle of brown liquid was not labeled.   This exposes employees to possible respiratory irritation and/or dermatitis hazards.
