In the wood shop, employees were exposed to amputation hazards, in that, there were no automatic adjusting hoods on SawStop table saws #1 and #2 while cutting wood or veneer, on or about 11/03/2016.

In the maintenance area, employees were exposed to a struck by hazard, in that, the Ryobi 6 inch abrasive wheel grinder's work rest was at 1/2 inch while used to sharpen drill bits and tools, on or about 11/03/2016.
