29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Lopez Roofing, ay 4172 Quebec St., Denver, CO 80237:  On and before November 3, 2016, the exposing employer did not ensure that employees wore safety glasses when using or working near pneumatic nail guns to while roofing this single family home.  Condition exposed employees to the hazard of projectiles to the eyes.

Condition exposed employees to fall hazards of up to approximately 15 feet.

Quebec St., Denver, CO 80237:  On and before November 3, 2016, the exposing employer did not ensure that ladders used to access the roofs extended at least 3 feet above the upper landing surface.  Condition exposed employees to potential fall hazards of up to approximately 10 feet as they transitioned from the ladder to the upper landing.
