On August 15, 2016 an employee entered the flat bottom sixty (60) foot diameter storage bin alone without the issuance of a permit in preparation for the sweep auger and was severely injured by the energized auger. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

On August 15, 2016 an employee entered the flat bottom sixty (60) foot diameter storage bin while the floor auger was operating and was severely injured by the energized auger. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

There was no observer stationed outside the bin on August 15, 2016 when an employee entered the flat bottom sixty (60) foot diameter storage bin to set-up the sweep auger and was exposed to an amputation hazard from the energized nine (9) inch floor auger. This violation was documented on November 4, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
