29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  Job Site:  192-196 Harvest Moon Court, Saint Peters, MO 63304, on or about 11/7/2016, workers were observed on top of an 4:12 pitch roof, actively installing new roofing shingles, operating pneumatic nail guns without any form of eye or face protection.  The workers were exposed to a struck-by hazard.

The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Job Site:  192-196 Harvest Moon Court, Saint Peters, MO 63304, on or about 11/7/2016, workers were observed using a Werner Aluminum Extension Ladder to gain access on and off the roof surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The workers were exposed to a fall hazard.
