29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) Tour Ed-Mine and Museum- 748 Bull Creek Road, Tarentum, PA: On or about November 7, 2016, the company did not develop an energy control program consisting of energy control procedures, employee training, and periodic inspections prior to conducting work on equipment such as but not limited to the 8-ton Ironton Locomotive.

29 CFR 1910.184(h)(1): Employer did not use natural and synthetic fiber-rope slings that have permanently affixed and legible identification markings stating the rated capacity for the type(s) of hitch(es) used and the angle upon which it is based, type of fiber material, and the number of legs if more than one:  (a) Tour Ed-Mine and Museum- 748 Bull Creek Road, Tarentum, PA:  On or about November 17, 2016, employees were using a synthetic sling to lift a 500lb motor into a locomotive; the sling's tag with the rated capacities were not legible at the time of the inspection.
