29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) During roofing and siding work: On or about 11/10/16, employees were exposed to fall hazard(s) of approximately 9' to 30' to ground/lower level while tearing off and replacing underlayment, flashings, roof shingles, and siding. Work was done on a steep roof with no fall protection system in use.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii) (a) During roofing and siding work: On or prior to 11/10/16, the employer did not ensure that employees were trained in the recognition and avoidance of hazards associated with work on a steep roof. Employees were observed tearing off and replacing underlayment, flashings, roof shingles, and siding with no fall protection system in use.
