OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to tip-over hazards:    (a) On or about November 02, 2016, and at times prior, Double Helix Construction, Inc. employees were exposed to tip-over hazards from operating a JLG Boom Lift, Model 80HX in a manner for which it was not intended by raising a 28 foot long, 128 lb. purlin on the platform railing approximately 21 feet high.      Reference JLG Operation and Safety Manual, sub-section 1-7 that states "Do not carry materials on platform railing unless approved by JLG Industries Inc." and sub-section 4-1 that states "It must not be used as a forklift, crane, support for overhead structure, or to push or pull another object."    Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
