29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a)  East side of the building , on or about November 09, 2016, two employees were working from a ladder jack scaffold without fall protection, thus exposing employees up to a 18-foot fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At the job site, on the roof, on or about November 09, 2016, threeloyees were observed working from the roof erecting wooden stands onto the roof deck without any means of fall protection, which exposed employees up to 15 feet fall hazards.
