An employee was exposed to fall and catapult hazards while working from the basket of an extensible boom aerial lift and was not protected by a personal fall arrest system, or fall restraint.

Location: Jobsite    Employees were exposed to fall hazards greater than 6 feet to the level below while working, where fall protection had not been provided.
