29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On or about November 3, 2016 and at times prior thereto at the worksite located at or near 7208 Huntington Court, Fort Smith, Arkansas,  employees were using pneumatic nail guns without safety glasses. This condition exposed the employee to struck by hazards to the eye.

On or about November 3, 2016 and at times prior thereto, at the worksite located at or near 7208 Huntington Court, Fort Smith, Arkansas, employees were engaged in residential home construction at heights greater than 6 feet without fall protection, exposed to the hazard of falling to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about November 3, 2016 and at times prior thereto, at the worksite located at or near 7208 Huntington Court, Fort Smith, Arkansas, employees were exposed to fall hazards routinely as part of their job installing siding and flashing on residential homes without having training in fall protection. This hazard exposed employees to the hazard of falling to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about November 3, 2016 and at times prior thereto, at the worksite located at or near 7208 Huntington Court, Fort Smith, Arkansas, employees access a second story roof using a portable extension ladder that did not extend at least three feet above the upper landing surface. This condition exposed employees to the hazard of falling to the ground below.

On or about November 3, 2016 and at times prior thereto, at the construction site located at or near 7208 Huntington Court, Fort Smith, Arkansas, an employee used a portable extension ladder to gain access to a second story window. The ladder was placed on a mound of dirt that was not level, exposing the employee to the hazard of falling to the ground below.
