29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a)  At 22850 Perdido Beach Blvd., Orange Beach, Alabama:  On or about November 15, 2016 and at times prior; the employer failed to ensure employees operating an aerial lift where wearing a harness and tied off exposing them to a fall hazard of 14 feet to the ground below.
