29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. The employer is failing to ensure that all employees exposed to eye and face hazards from flying particles while working on a residential roof. This most recently occurred on November 15, 2016 on the jobsite located at 515 W. Saunders Ave, Lincoln, NE 68521. Two employees were observed using pneumatic tools without any form of eye or face protection while installing roofing material. One employee was using a pneumatic hand saw and another employee was using a pneumatic nail gun.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection. This most recently occurred on November 15, 2016 on the jobsite located at 515 W. Saunders Ave, Lincoln, NE 68521. Employees were observed reroofing the residence on a 5 in 12 steep sloped roof at heights of approximately 20 feet above the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof. This most recently occurred on November 15, 2016 on the jobsite located at 515 W. Saunders Ave, Lincoln, NE 68521. Employees are using a ladder to enter and exit the roof to perform reroofing duties and did not have the side rails extended to at least 3 feet above the landing surface. The landing surface is approximately 20 feet above the ground.
