29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:      Westborough Reservoir Estates, Westborough, Massachusetts; On or before November 15, 2016 employees were not provided a safe means to exit trench excavations that were more than 4 feet deep.  The employer had a wooden A frame ladder on site for employees to use to exit the trenches.

29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face:    Westborough Reservoir Estates, Westborough, Massachusetts;  On or before November 15, 2016 employees working inside of trenches were exposed to struck by hazards from rocks and stones falling from the excavation face when installing piping and concrete manhole structures.

Westborough Reservoir Estates, Westborough, Massachusetts; On or before November 15, 2016, employees installing concrete drainage piping and manhole structures were exposed to cave-in hazards while working in unprotected trenches greater than 5 feet deep.
