The employer did not provide training to the employees to recognize the hazards associated with fall protection, exposing employees up to a 10 foot fall hazard.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access. a) On or about November 16, 2016 and at times prior to, employees performing work related to roofing used an extension ladder to gain access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface or secured at the top to a rigid support, exposing the employees to fall hazard of about 20 feet.
