29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  On or about November 8, 2016 an employee fell approximately 15 feet from a ladder after an adjacent ladder was blown over when an employee left it unattended in windy conditions and a competent person did not recognize the hazard and take prompt corrective measures to eliminate the hazard.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:  On or about November 8, 2016 an employee suffered injuries from a fall from a ladder after an adjacent ladder was blown over in windy conditions and the employer did not provide the training necessary to recognize hazards associated with the proper use and placement of ladders in windy conditions.  The CHADCO Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1060(a)(1)(iii), which was contained in OSHA inspection number 1081601, citation number 1, item number 1b and was affirmed as a final order on September 21, 2015, with respect to a workplace located at 285 E Broadway, Roseville, IL 61473.
