29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The controlling contractor is failing to ensure that all employees of the sub-contractor Murillo Roofing exposed to fall hazards while working on a residential roof are utilizing fall protection. This most recently occurred on November 15, 2016 on the jobsite located at 6537 S. 19th St, Lincoln, NE 68512. Workers were observed reroofing the residence on an 8 in 12 steep sloped roof at heights of approximately 12 feet above the ground.
