29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    Worksite located at 101 W 8th St. Tishomingo, OK 73460. On or about November 23, 2016 employees were performing roofing work without the use of fall protection and exposed to a fall hazard of more than 6 feet.

On or about November 23, 2016 employees were using a portable ladder to gain access to the roof and the ladders side rails did not extend 3 feet above the landing surface, exposing the employees to a fall hazard of more than 6 feet.
