29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment:      On or about November 15, 2016, Employees operating a orderpicker lift truck were exposed to a fall hazard when using fall protection because the lifeline were not appropriate, and the employer had not provided adequate training on the fall protection.      The training shall include:    a.      When personal protective equipment (PPE) is necessary,    b.      What PPE is necessary,    c.       How to properly don, doff, adjust and wear PPE,    d.      The limitations of the PPE and   e.      The proper care, maintenance, useful life and disposal of the PPE.
