(a)    On or about 10/28/16, at 2344 Lyle Avenue at the former MAC's Pizzeria and Grill; workers were exposed tripping, falling and stepping into open holes in the floor area where renovation work was being performed.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purpose for which they were designed:     (a)    On or about 10/28/16, at 2344 Lyle Avenue at the former MAC's Pizzeria and Grill; a ten foot A-Frame stepladder, designed to be used in the open and braced position, was left in the closed position while an employee utilized it to apply mortar to the top edge of a north facing wall.    NO ABATEMENT CERTIFICATION REQUIRED
