The platform was set on the outside of the ladders.  The employer did not provide safe access to the scaffold platform.   (c)  On or about November 29, 2016, employees were exposed to a fall of approximately 22 feet.  The employees were installing siding materials on an existing residential home located in Cleveland, Ohio 44012.  The employees were working from a ladder jack scaffold.  The employees were not using any form of fall protection.
