29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:     On or about November 25, 2016, during the installation of structural framing material (plywood sheeting/decking) at a job site located at 100 N. Ave, Tallmadge, Ohio 44278; the employer did not ensure employees working on the 4th floor installing sheeting/decking on the floor joists were protected from a fall of approximately 20.4 feet by utilizing a fall protection system.
