29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a.  In the maintenance shop, on or about 11/17/16:  An employee was working underneath a Western-Star auto hauler while the truck's front wheels were placed on two 8x8 wooden ramps.  The truck's brakes were not set, the transmission was in neutral and the wheels were chocked in only one direction.  The truck rolled off the ramps and its frame pinned the employee to the ground causing substantial injury.  Lockout/tagout (LOTO) procedures were not developed nor utilized.  Abatement certification must be submitted for this item.
