Where there is more than one location to be reached from a single set-up position, either individual trial lifts for each location, or a single trial lift, in which the platform is moved sequentially to each location, must be performed; the method selected must be the same as the method that will be used to hoist the personnel.  a. Located at 14305 East Broad Street in Pataskala, Ohio: On or about December 1, 2016, the employer had employees hoist a contractor's technician in a personnel basket with a Grove telescoping mobile truck crane, model AT990, serial 994016. The employer did not ensure that a trial lift was performed prior to hoisting the technician, thereby exposing the technician to a fall hazard of approximately 120 foot.
