Machine shop, interior wall; on or about 29 November 2016 and times prior thereto; an energized two-place electrical outlet box had an Ace six-way receptacle splitter attached to a 25-foot orange heavy duty extension cord, which is  routed to a three foot, six-place wall mounted relocatable power tap; Ace model number 33543. The relocatable power tap is used to distributed power to five Makita model DC18RA, 3.0 Amp battery charging stations. A 25-foot orange extension cord attached to the relocatable power tap is used to provide power to a three receptacle splitter, which is used to provide power to three additional Makita model DC1414, 2.6 amp battery charging stations. This condition exposed employees to a fire hazard.

South bay, cutting area, above the Siemens Main Breaker Panel; an energized four-place electrical outlet box was missing a knockout plug on the left hand side.       i. South bay, above door adjacent to the cutting area; two knockout plugs were missing from the bottom of an energized electrical box.       j. West bay, adjacent to the entry way; an energized Siemens G1224ML1125, Type 1,  12-place Main Breaker Panel had two knockout plates, measuring approximately 1-inch by 2 and 1/4  inches on, spaces 9 and 11 removed and covered with blue duct tape.       k. West bay, adjacent to the entry way and to the left of the Siemens Main Breaker Panel; an energized single place light switch was missing a knockout plug on the right hand side.       l. West bay, south wall; an energized two-place electrical outlet box, with a number four printed on the right hand side, was missing a knockout plug on the right hand side.       m. West bay, to the left of the Challenger Main Breaker Panel; an energized two-place electrical outlet was missing a knockout plug on the left hand side.        n. Machine shop, adjacent to the passage way; an energized Siemens P4260B1225CU, Type 1, 42-place Main Breaker Panel had a knockout plate, measuring approximately 1-inch by 2 and 1/4 inches missing on space number 30.       o. Machine shop, just above and to the left of the Siemens Main Breaker Panel; an energized electrical box was missing the cover plate. Outlet box located approximately seven feet from the lower surface.       p. Machine shop, exterior door entrance, adjacent to roll-up door; an energized light switch was missing a knockout plug on the top of the outlet box.       q. East bay, adjacent to entryway to the machine shop; an energized Challenger SL20, Type 1, 40-place Main Breaker Panel was missing a knockout plug on the lower right hand side.       r. East bay, adjacent to the exit; an energized two-place electrical switch was missing the rocker switch on the right hand side. Opening was covered with duct tape.       s. East bay, interior wall; and energize outlet with RSP printed on left hand side was missing a knockout plug on the left hand side.       t. Wood shop to the left of the doorway; an energized 240-volt outlet was missing a knockout plug on the right hand side.       u. South Shop, adjacent to air compressor; an energized electrical outlet box was missing the cover plate.    This condition exposed employees to an electrical shock and electrical burn hazards.

The power cord terminal fitting for the Emerson, model SA55NXSBT-2759, AC motor was separated from the switch box.       b. South bay, an energized electrical outlet box providing power for a Miller Welder unit was not provided with strain relief. The outlet box was missing the terminal fitting.        c. Throughout the shop bay areas; on or about 29 November 2016 and times prior thereto; the employer did not ensure that portable cord and plug connected equipment and flexible cord sets (extension cords) were visually inspected before use. Six instances were noted:       a. South bay, cutting area; an energized Miller XMT 304 CC/CU DC Inverter Arc Welder, serial number M8250511A, had a damaged power cord. Outer protective jacket was separated from the plug.        b. South bay, cutting area; an energized Miller XMT 304 CC/CU DC Inverter Arc Welder, serial number unknown, had a damaged power cord. Outer protective jacket was separated from the plug.        c. South bay, cutting area, ceiling mounted; an energized Reelcraft 40-foot, three place retractable reel cord had damaged to the outer protective jacket at the receptacle plug.        d. West bay, adjacent to shelving; a 25-foot heavy duty black extension cord had damaged to the outer protective jacket on multiple locations along the length of the cord.        e. South shop, work bench; an energized Hitachi G12SE2, Serial number MN20112, 4 1/2 inch,  Disc Grinder had damage to the power cord outer protective jacket.        f. West bay; an energized Jet 18-inch Woodworking Bandsaw, Model JWBS-18QT-3, Serial number 1001564, had damage to the power cord outer protective jacket adjacent to the plug.   This condition exposed employees to an electrical shock and electrical burn hazards.
