29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace:    (a) Bright Pond Road:  On or about November 29, 2016 and times prior, two employees wearing a set of leg chaps that were cut and/or torn exposing the internal cut resistant material.      (b) Bright Pond Road:  On or about November 29, 2016 and times prior, an employee was wearing a set of leg chaps that were torn and the cut resistant material wasn't evenly dispersed throughout the leg protection collecting at the bottom of each leg.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Bright Pond Road:  On or about November 29, 2016, and times prior, employees were not protected from flying objects, such as wood, wood chips, needles and splinters by appropriate face protection when using gas powered chainsaws, such as a Stihl MS261C with an 18in. bar and a Husqvarna 545 with an 18in. bar to clear brush from a 300ft. power line right away.
