29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board. On or about December 1, 2016, the employer did not ensure that employees working from an elevated, open sided platform at the mold machines, including number 17 and 18, were protected from falls onto or around dangerous equipment. Abatement certification is required in for this item accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about December 1, 2016, employees were exposed to caught between and struck by hazards while operating the following production equipment and the employer did not ensure that rotating components were adequately guarded: (a) The Milltronics CNC RH-20 had an unguarded tool. (b) The ToolMex (TUR 710, S#50636), Sigma (LT-05), and OKUMA (L-S, S#4608-3413) lathes were missing chuck guards. (c) The Milltronics CNC Lathe, ML-22 (S#12782), had a missing side panel and the interlock was not properly functioning Abatement certification is required in for this item accordance with the requirements of 29 CFR 1903.19(c).
