29 CFR 1926.651(c)(2):  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    (a) ECI Site Construction Management, Inc. at 2300 15th Street, Denver, CO 80202:  On or before December 2, 2016 the employer did not ensure entry and egress from an excavation was provided at no more than 25 feet of lateral travel.  Three ladders were placed inside the excavation, which was approximately 200 feet in length.  This condition exposed employees to lack of egress in the event of a trench cave-in.

29 CFR 1926.651(j)(2):  Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:    (a) ECI Site Construction Management, Inc. at 2300 15th Street, Denver, CO 80202:  On or before December 2, 2016 the employer did not ensure that the spoil pile was at least two feet from the edge of the excavation, which was approximately 7 feet deep, 20 feet wide at the top, and 200 feet in length. This condition exposed employees to the hazard of soil and rocks falling into the trench and/or causing trench wall collapse.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):       (a) ECI Site Construction Management, Inc. at 2300 15th Street, Denver, CO 80202:  On or before December 2, 2016 the employer did not ensure that each employee working inside an excavation was protected from cave-in, in that the excavation, which was approximately 7 feet deep, 20 feet wide at the top, and 200 feet in length had near vertical side walls in the section by the piped-water return area.  This condition exposed employees to a crushing hazard by excavation wall cave-in.
