29 CFR 1910.213(h)(1): The upper hood shall completely enclose the upper portion of the blade down to a point that will include the end of the saw arbor. The upper hood shall be constructed in such a manner and of such material that it will protect the operator from flying splinters, broken saw teeth, etc., and will deflect sawdust away from the operator. The sides of the lower exposed portion of the blade shall be guarded to the full diameter of the blade by a device that will automatically adjust itself to the thickness of the stock and remain in contact with stock being cut to give maximum protection possible for the operation being performed:     On 12/05/2016, employees from Central Pattern Company located at 8830 Pershall Road, Hazelwood, MO, were exposed to hazards at the point of operation created by rotating saw blade.  The Sears Craftsman radial arm saw had approximately ½ inch of the saw blade unguarded at the lower portion of its blade and at the point of operation.  This lower exposed portion of the blade was not adequately guarded to the full diameter of the blade by its device that will automatically adjust itself.  This saw was located in the South end of the shop in the Shipping and Receiving Department.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.):    On 12/05/2016, employee(s) from Central Pattern Company located at 8830 Pershall Road, Hazelwood, MO were exposed to struck-by hazards.   The tongue guards on a Sears Craftsman bench grinder were not adjusted to within one-fourth inch.  This bench grinder was located in the Central West side of the shop.

29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance:   On 12/05/2016, Central Pattern Company located at 8830 Pershall Road, Hazelwood, MO had not adequately developed, documented and utilized proper procedures for the control of potentially hazardous energy.  The following is a list of machines located in the plant requiring an adequately written procedure:  The HAAS VF-6 Milling Machine (at least electrics and pneumatics);  and the OKUMA MA-60HB (at least electrics and pneumatics).
