29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets.  (a) At the site employees were installing a 4/12 pitch replacement roof while exposed to a 8 foot fall hazard without fall protection on or about 12/7/2016.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) At the site employees were replacing a 4/12 pitch roof while exposed to an 8 foot fall hazard where the employer failed to ensure that the employees obtained fall protection training on or about 12/7/2017.
