29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    (a) Construction site - On or about December 7, 2016, an employee working on top of a IT beam (spandrel) had fall protection connected at his feet using a tie strap that was connected to his lanyard and the spandrel was 12 feet above the concrete floor. The employee's feet would strike the concrete floor below a shock absorber of his lanyard would deploy.
