29 CFR 1926.403(b)(2):  Listed, labeled, and/or certified equipment was not installed and/or used in accordance with instructions included in the listing, labeling, and/or certification:  711 Boston Rd., Springfield, MA: On or about December 7, 2016, the following electrical devices were used in a manner inconsistent with their labeling, listing and/or certification:   a) a Defiant brand GFCI was powered by an extension cord. b) a Hilti brand GFCI was powered by an extension cord. c) extension cords were daisy chained together to extend their span.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold is used in a stationary manner:  a) 711 Boston Rd., Springfield, MA: On or about December 7, 2016, an employee was working from a Bakers scaffold whose wheels were not locked as the scaffold was being used in a stationary fashion.
