(a) Runnymeade Road:  On December 5, 2016, an employee was exposed to a fall of approximately 100ft. due to improper use of the double leg lanyard while transitioning from the top of a fixed ladder onto the working platform of an advertising billboard to change out the vinyl sign, and did not ensure at least one leg of the lanyard remained attached to an anchor point at all times.    (b) Runnymeade Road:  On December 5, 2016, an employee using a vertical lifeline fall protection system to climb a fixed ladder of a billboard advertising sign had to manually move or drag the rope grab as he climbed because the fall protection components provided to the employee, by the employer, were not used properly or approved to be used together by the manufacturer.
