29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a) Jobsite, 110 Route 30, North Huntingdon, PA:  On or about December 7, 2016, three employees were installing shingles on a low sloped commercial roof and were not protected from falling.  The fall hazard to the employees was approximately 18 to 25 feet to the ground.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite, 110 Route 30, North Huntingdon, PA:  On or about December 7, 2016, three employees were working on a commercial roof without fall protection and were exposed to falls of approximately 18-25 feet to the ground.  The owner said the employees had been trained, but did not have any written documentation of it.
