29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    (a) Facility, Blow Mold Machine 5: On or before August 25, 2016, guarding which was installed to protect employees from the de-tabbing process did not protect access to the area in the horizontal plane at the bottom of the hinged guard.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a) Facility, Blow Mold Machine 7: On or before December 5, 2016, employees were exposed to fall hazards of approximately 7 feet 10 inches in that personal fall protection equipment was not used while changing elevated blow molding machine filters.
