29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.   At this workplace, an employee conducted powered industrial truck maintenance operations and the employer did not develop and implement energy control procedures for securing the mast to prevent an uncontrolled  release.  This exposed employees to amputation hazards.

At this work place, employees operated powered industrial trucks for cotton seed oil milling operations and were in need of repair which were not taken out of service such as but not limited to:   a. 2005 Nissan Model MAPL02A25LV- Needs tires; b. 2007 Toyota- No backup alarm; c. 2005 Nissan Model MAPL02A25PV- No tail lights; d. 2005 Nissan Model KCPH)2A25PV- Need tires, strobe, tail lights;   e. 1998 Toyota Model 426FGU25-No horn or strobe.

At this work place, employees conducted cottonseed oil milling operations and rotating machinery was not guarded throughout the facility in areas such as but not limited to:  a. the rotating saw shafts (51 each); b. the fan shafts (27 each); c. the float bearings (51 each).
