29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved:    (a) Location - 451 Boston road, Billerica, MA:  On or about 12/9/16, the employer failed to ensure that safety related work practices (consistent with the nature and extent of the associated electrical hazard) were used by employees while they were working on energized electric.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2):  (a) Location - 451 Boston Road, Billerica, MA: On or about 12/9/16, the employer failed to ensure employees were locking or tagging energized electrical circuits. The employer failed to ensure employees were protected from energized electrical parts while replacing lighting ballast.
