29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.  On or about December 7, 2016, employees of The Shield Construction accessed a deck by using a ladder that did not extend 3 feet above the landing, exposing employees to a 10 feet fall hazard.

On or about December 7, 2016, on the west side of the deck, employees of The Shield Construction were using a folded up step ladder to access the deck, were exposed to a 10 feet fall hazard.
