29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 12432 Squirrel Dr. Lot 81, Spanish Fort, Alabama:  On or about December 12, 2016 and at times prior; the employer allowed his employee to install roofing paper on a high slope roof without fall protection exposing his employees to a fall hazard of 9 feet 7 inches to the ground below.

29 CFR 1926.1053(a)(1)(ii): Portable ladders that were not self-supporting, without failure, at least four times the maximum intended load:  a)  At 12432 Squirrel Dr. Lot 81, Spanish Fort, Alabama:  On or about December 12, 2016 and at times prior;  the employee allowed his employees to use a job made ladder to gain access to the roof and the rungs of the ladder did not blocks installed exposing employees to a fall hazard of 9 feet 7 inches to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 12432 Squirrel Dr. Lot 81, Spanish Fort, Alabama:  On or about December 12, 2016 and at times prior; the employer failed to ensure a job made ladder used by employees to gain access to the roof top had its rails extend at least three feet above the landing area exposing his employees to a fall hazard of 9 feet 7 inches to the ground below.
