29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  a) Jobsite  On or about December 9, 2016, employees were allowed to use electric hand power tools without using  eye protection such as safety glasses, as a result exposing employees to ricocheting and flying debris hazards.   1. 29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) Jobsite-On or about December 9, 2016, an employee was using a yellow 12 AWG extension cord without a ground prong attached.  The extension cord with missing ground supplied power to double insulated portable electric circular saw (Makita 5007F), exposing an employee to a shock and electrical hazard.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) Jobsite - On or about December 9, 2016, employees were using a yellow 12 AWG extension cord that was used to supply power to 120 V electric circular saw (Makita 5007F) black power cord without strain-relief in that the outside protective sheath was pulled from the neck, exposing employees to electric and shock hazards.    b) Jobsite - On or about December 9, 2016, employees were using a orange 12 AWG extension cord that was used to supply power to 120 V electric Milwaukee Hammer drill (Milwaukee Hammer Drill S/N G43BD160303130) black power cord without strain-relief at the temporary power pole, in that the outside protective sheath was pulled from the neck, exposing employees to electric and shock hazards.
