A.  On or about December 16, 2016, Electrical Assembly Area:  The front edge of a storage mezzanine above an office area was not provided with a standard guardrail.  The opening measured 8 feet, 9 inches along the face and exposed employees to falls of 9 feet to the concrete floor below.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy   A.  On or about December 16, 2016, Electrical Assembly Area:  A four-receptacle outlet box on the wall behind the electrical assembly bench was missing a faceplate, exposing the live electrical parts.  The outlets were energized and provided power to a blue extension cord, which in turn powered a blue extension cord, which powered a three way splitter, which in turn provided power to an orange extension cord and two relocatable power taps which energized various electrical equipment in the area.

The relocatable power tap was listed to be powered directly through a wall outlet.      2.  Main Production Area:     a.  A six-outlet Wiremold relocatable power tap was energized through an orange extension cord.  The relocatable power tap was listed to be powered directly through a wall outlet.

On or about December 16, 2016, in the following instances, flexible extension cords were used in lieu of the fixed wiring of the building:  1.  Electronic Assembly Area:       a.  A blue extension cord was used to power a yellow cordset to a three way splitter.     b. The three-way splitter powered a four-outlet relocatable power tap on the ground underneath the electrical assembly bench, which powered a computer and de-soldering station.     c. The three-way splitter powered an orange extension cord which was available for use.     d.  The three-way splitter powered a six-outlet relocatable power tap on the electrical assembly bench, which powered a laptop computer, charger, inspection lamp, plasma lamp and speaker.     2.  Main Production Area:     a. An orange extension cord was used in lieu of the fixed wiring of the building to energize a Wiremold six-outlet relocatable power tap, which was used to energize a computer and peripherals.
