29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 50 parts per million:  a) At the facility, on or about 12/12/2016: An employee was exposed to levels of carbon monoxide exceeding the Permissible Exposure Limit. An employee was exposed to carbon monoxide at an 8 hour time weighted average concentration of 84.5 parts per million, 1.69 times the Permissible Exposure Limit over a time period of 330 minutes. Zero exposure was calculated for the remaining 150 minutes.  b) At the facility, on or about 12/12/2016: An employee was exposed to levels of carbon monoxide exceeding the Permissible Exposure Limit. An employee was exposed to carbon monoxide at an 8 hour time weighted average concentration of 128.8 parts per million, 2.58 times the Permissible Exposure Limit over a time period of 330 minutes. Zero exposure was calculated for the remaining 150 minutes.  c) At the facility, on or about 12/12/2016: An employee was exposed to levels of carbon monoxide exceeding the Permissible Exposure Limit. An employee was exposed to carbon monoxide at an 8 hour time weighted average concentration of 67.1 parts per million, 1.34 times the Permissible Exposure Limit over a time period of 270 minutes. Zero exposure was calculated for the remaining 210 minutes.   Abatement certification must be submitted for this item.
