29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  On or about January 12, 2017, at the above addressed jobsite, employees were conducting construction activities including restorations and cleaning. Employees were using chemicals that exposed them to inhalation hazards and/or required the use of respiratory protection. The employer failed to provide medical evaluations and respirator fit testing to the exposed employees.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:  On or about January 12, 2017, at the above addressed jobsite, employees were conducting construction activities including restorations and cleaning. Employees were working with chemicals and / or cleaning compounds requiring personal protective equipment to safeguard against skin irritation and / or inhalation hazards. The employer failed to provide employees with a written hazard communication program containing all of the elements listed above, in accordance with this standard.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  On or about January 12, 2017, at the above addressed jobsite, employees were conducting construction activities including restorations and cleaning. Employees were working with chemicals and / or cleaning compounds requiring personal protective equipment to safeguard against skin irritation and / or inhalation hazards. The employer failed to provide employees with effective information and training on hazard chemicals in their work area in accordance with this standard. On or about January 12, 2017, at the above addressed jobsite, employees were conducting construction activities including restorations and cleaning. Employees were working with chemicals and / or cleaning compounds requiring personal protective equipment to safeguard against skin irritation and / or inhalation hazards. Employees also worked from ladders / scaffolding / other working surfaces that require specific training and / or fall protection. The employer failed to provide employees with a written certification that a hazard assessment was conducted in accordance with this standard.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(c).
