The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  On or about December 17, 2016, Blow Mold Technicians were exposed to machine hazards associated with rotating parts while reattaching the #2 blow mold machine cyclone air lock guard.  The employer did not ensure Blow Mold Technicians implemented energy control procedures by turning off the machine [per the 1910.147(d)(2) requirements], isolating the machine from the energy source(s) [(d)(3)], affixing a lockout device [(d)(4)(i)], and verifying de-energization and isolation had been accomplished prior to cleaning the machine [(d)(6)].  Energy sources include electric power.
